Test Preparations

MTM Lab Tests & Screening staff ensures a safe and comfortable environment to make your testing collection process as secure, quick, and relaxed as possible while protecting your privacy. Whether you need to test for employment, legal, or personal reasons, we offer a range of testing options to meet your needs.

When visiting our lab, you should bring:

• The CCF test request form from a health care professional/ employer/ agency requesting the laboratory testing. 
• A photo ID, Drivers license or employee identification badge.
• Form of payment credit card, debit card, zelle or apple pay. 

NOTE: You may not bring children in the collection area unless being served and not at all during any drug screening procedure.
