At MTM Lab Tests & Screening, our staff is dedicated to ensuring a safe and comfortable environment, making your testing collection process secure, quick, and relaxed while safeguarding your privacy. Whether you require drug screening for employment, legal, or personal reasons, we provide a variety of laboratory testing options to suit your needs.

When visiting our lab, please remember to bring:

- The CCF test request form from a health care professional, employer, or agency requesting the laboratory testing.

- A photo ID, driver’s license, or employee identification badge.

- A form of payment such as a credit card, debit card, Zelle, or Apple Pay.

NOTE: Children are not permitted in the collection area unless they are being served, and they should not be present during any drug screening procedure.
